A note on sensitive information: The “Main Care Concerns” field may include health-related details about a prospective resident. We collect this information only because you choose to provide it, and only to help us evaluate whether our home may be an appropriate fit and to prepare for a possible tour or admission. We do not use this information for any purpose beyond responding to your inquiry.
